STORAGE PRICING:

• $12 to receive a kit and store your DNA at home for 1 kit with 1 FTA Elute card

• $80 per person to store your DNA at DNAPrint® for 5 years with 2 FTA Elute cards

ORDER PROCESS

1. To order using paypal or a credit card, click on the "Add to Cart" buttons above. To process your order via mail or fax, simply download this order form and complete it.

2. If it is a mail order, mail the completed order form with a check or money order to DNAPrint®. If it is a fax, fax the order form with your credit card information to DNAPrint®.  The contact information is on the order form.
3. When we receive your order and payment, we will then send you a kit.
4. If you are storing DNA at home, your kit will contain:
• Swab
• 1 FTA Elute card
• Cheek swabbing instructions

Swab the cheeks and transfer the DNA onto the FTA Elute card according to the cheek swabbing instructions.  You will store the FTA Elute card in a safe location at your home.

6. If you are storing your DNA at DNAPrint®, your kit will contain:
• 2 swabs
• 2 FTA Elute cards
• Return envelope for FTA Elute cards
• DNA storage information and consent form
• Cheek swabbing instructions
• Access authorization form

Swab the cheeks and transfer the DNA onto the FTA Elute card according to the cheek swabbing instructions.  Place the cards in the return envelope and mail the following items back to DNAPrint®:

• 2 FTA Elute cards
• Signed DNA storage information and consent form
• Completed and signed access authorization form

7. If you are ordering an STRMAP analysis, your kit will contain:

• 2 swabs and swab return envelope
• DNA consent form
• Cheek swabbing instructions

Swab the cheeks in accordance with the cheek swabbing instructions and put the 2 swabs in the swab envelope. Please return the swab envelope and consent form back to DNAPrint®.  Your STRMAP report will be sent to you within 4-5 weeks after the swabs are received by DNAPrint®.
